During your first visit, Dr. Guerrero will explain the diagnosis and the treatment options. Treatment may be performed on the day of your consultation. However, in case of any serious complications, your doctor may suggest for second visit to provide the treatment on another day.
Please provide the following details at the time of initial consultation:
- Completely filled referral form
- X-rays if applicable
- List of medication you are currently taking
- Your medical or dental insurance. It helps to you get all the forms filled completely. This will not only save the time, but also help process the claims.
Note: Patients who are under 18 years should accompany with a parent or guardian during the consultation visit.
Also, please inform our staff if you have any of the following conditions before having the surgery.
- Diabetes
- High blood pressure
- Rheumatic fever
- Artificial heart valve or join
If you are on any of these medications, such as:
- Aspirin
- Heart medications
- Anticoagulant therapy
- Or taking for any other special conditions
Get if you have taken any X-ray within the last six months or ask your doctor to forward it to our dental office. In case of any additional x-rays are needed, they will be taken at our facility.
